The bobblehead is sponsored by Mercury Insurance.  The bobblehead was part of 4 box series.  On the box it lists some of Anze's favorite things.  The bobblehead also includes a bobblehead and likeness of Anze's current household pet, Gustl.  The bobblehead is missing Kopitar's hockey stick, but the item and autograph are still in excellent condition and is perfect for display purposes.
